In my letter I mentioned that I contacted the Civil Status Office and
the Ministry of Technology, Communication & Innovation for information
regarding the "consent form". I did not ask her an advice on the matter
though.

I only asked her about the fingerprint minutiae that is said to be
stored in the chip of the ID card.

I will look at the "para 5" you mentioned.
